Your Case Plan

Each year beginning at age 14 your team will help create your own case plan—which is a plan to make sure all of your needs are being met and you are getting tools to help you get ready for being an adult. Some of these needs include your physical health (going to the doctor, dentist, orthodontist), your mental health (going to therapy) and making sure you are learning independent living skills (like housework, budgeting, getting your first job.) Your case plan must have a list of rights for education, health, visitation, going to court, and safety.
